# MediLedger Blockchain Network for Pharmaceutical Supply Chains

> MediLedger is an operational blockchain network developed by Chronicled since 2017 to make pharmaceutical supply chains transparent and combat counterfeiting. The network launched in 2019 and successfully completed an FDA pilot for DSCSA compliance in 2023, involving seven of the ten largest pharmaceutical companies and all three major US wholesalers. The solution significantly reduces the verification of drug returns, which conventionally takes up to 48 hours, through blockchain technology and barcode scanners.

## Description

Pharmaceutical chargeback billing is a $500+ billion process in the USA: pharmaceutical manufacturers sell medicines to wholesalers at list price; wholesalers sell to hospitals and clinics at contractually agreed lower prices; wholesalers claim the difference back as chargebacks from the manufacturer.

The problem: hospitals have individual contract prices (GPO contracts, direct contracts) that constantly change. Wholesalers must document these prices correctly and calculate chargebacks accurately. Up to 5% of all chargeback transactions contain errors — with a volume of $500 billion, that is $25 billion potential errors per year. Manual rework of chargeback disputes is time-consuming and expensive. MediLedger solves this with a blockchain-based chargeback engine: price contracts are stored as smart contracts on the blockchain; when a transaction is carried out, the smart contract automatically checks the correct price; the chargeback amount is automatically calculated and triggered. Drug Patent Watch analysed MediLedger as a practical example of real blockchain use in the pharmaceutical industry. An NCBI PMC article (2024) confirms the efficiency of blockchain-based drug traceability and billing processes.

## Benefits

- Pharmaceutical companies reduce disputes and administrative costs through automated contract and chargeback processes.
- Verification of drug returns is reduced from up to 48 hours to a few minutes through automated blockchain processes.
- The system offers immutable documentation of all transactions in the pharmaceutical supply chain, thereby preventing counterfeiting.
- Compliance with regulatory requirements such as the US DSCSA is automated and simplified.
- Compliance and supply chain employees can focus on strategic tasks as repetitive verification processes are automated.

## Challenges

- Companies must invest in new infrastructure and employee training to successfully implement blockchain technology.
- Coordinating all supply chain actors requires significant alignment and common standards between competing companies.
- Integration with existing legacy systems and ERP solutions poses high technical demands on implementation.
- Employees must acquire new skills in handling blockchain-based systems and adapt their working methods to digital processes.

## Technology foundation

MediLedger uses a decentralized blockchain network focused on interoperability and anonymisation of data between participants in the pharmaceutical supply chain. The technology enables immutable documentation of transactions and product movements while keeping request and response times under 400 milliseconds, which is necessary for operational use.
