## Description

The concept of the patient-controlled health record (PCHR) addresses the data sovereignty issue in healthcare: today, clinics store patient data in proprietary systems; patients have no central access and no control over data sharing.

A systematic review in Frontiers in Blockchain (2026) examined the entire state of blockchain-based healthcare applications. Result: PCHR systems are among the most advanced and practical applications. The concept: the patient creates a self-controlled digital health record on the blockchain; clinics, labs, and pharmacies write findings into this record (with patient consent); the patient decides via smart contract who may access which data. Every access is immutably logged. Advantages over central EHR systems: (1) No single point of failure; (2) No data loss when practices close; (3) Complete patient history without redundancy issues; (4) Immediate access for emergency doctors with patient consent. EU regulation: GDPR Art. 20 requires data portability in healthcare — blockchain-based PCHR is a natural technical implementation.
