MediLedger: Blockchain-based Pharmaceutical Traceability

MediLedger is an industry-led blockchain project that improves the traceability and authenticity of pharmaceuticals in the supply chain from manufacturer to dispenser. The system reduces verification time for returned medications from up to 48 hours to less than 400 milliseconds and supports compliance with the Drug Supply Chain Security Act (DSCSA). Since 2019, the network has been commercially operational and was successfully tested in a comprehensive FDA pilot project in 2023.

Description

MediLedger is a Hyperledger Fabric-based blockchain platform for DSCSA compliance, developed by Chronicled (San Francisco). The FDA pilot report (available on fda.gov) documents the technical architecture in detail: each medication unit is managed as a non-fungible token; ownership changes via smart contract; zk-SNARKs protect proprietary pricing data.

The DSCSA (Drug Supply Chain Security Act, 2013) mandates phased full serialization and interoperable traceability of all US prescription drugs. Full compliance became mandatory in November 2023. MediLedger architecture: each package receives a serial number; upon shipping, the manufacturer creates a blockchain entry (EPCIS standard); each ownership change (manufacturer→wholesaler→pharmacy) is documented as a smart contract transaction. zk-SNARKs (Zero-Knowledge Succinct Non-Interactive Arguments of Knowledge): compliance checks can be performed without revealing price negotiation or quantity data. Performance test in the FDA pilot: 2,000 transactions per second achieved — sufficient for the US pharmaceutical industry.
